Potassium and Magnesium Replacement Protocol

POTASSIUM AND MAGNESIUM REPLACEMENT PROTOCOL
For patients with a measured serum creatinine below 1.5 mg/dl
and whose serum creatinine has not increased by more than 50% from baseline:
‘ MONITORED INTRAVENOUS POTASSIUM REPLACEMENT (The patient must have a central line
and be in a monitored bed)
For serum K+ ≤3.0, give KCL 60 meq IVPB over 3 hours.
For serum K+ 3.1 – 3.5, give KCL 40 meq IVPB over 2 hours.
For serum K+ 3.6 – 3.9, give KCL 20 meq IVPB over 1 hours
‘ STANDARD INTRAVENOUS POTASSIUM REPLACEMENT
For serum K+ ≤3.5, give KCL 10 meq per hour x 4 hours (Total dose = 40 mEq)
For serum K+ 3.6 – 3.9, give KCL 10 meq per hour x 2 hours (Total dose = 20 mEq)
A. Recheck serum K+ 30 minutes after replacement is infused and repeat the above as required.
B. When administered via a peripheral line lidocaine 20mg (provided no documented allergies) may be
added to the K+ rider if the patient experiences burning or other irritation after slowing the rate of
administration. Note: Replacement is no greater then 10 meq per hour per peripheral line.
‘ MAGNESIUM REPLACEMENT
For serum magnesium below 1.5, start Magnesium Sulfate 2 GM/50ml D5W to run over 1 hour.
For serum magnesium 1.5 to 2.2 , administer Magnesium Sulfate 1 Gm/50 ml D5W over 2 hours
For serum magnesium >2.2, No replacement
C. Recheck serum magnesium one hour after the replacement is infused and repeat the above as required
